Welcome to Sol Midwifery Services (“midwife”, “practice”, “we,” “us,” or “our”). These Terms of Service (“Terms”) govern your use of our services, website, and digital communications. By scheduling an appointment, submitting information through our website, or visiting our office, you agree to be bound by these Terms.


1. Scope of Services

We provide evidence-based, holistic midwifery care including prenatal visits, labor and birth support, postpartum care, and newborn assessments. Services will be provided in-office and at your home.

Please note: Wellness care is not a substitute for emergency or urgent medical care. In the event of a medical emergency, call 911 or visit the nearest emergency department.


2. Eligibility

You must be at least 18 years old to consent to your own care. A parent or legal guardian must provide consent for any patient under the age of 18. We may request documentation to confirm legal guardianship when necessary.

Midwifery care is appropriate for low-risk pregnancies. Clients with high-risk conditions or complications may be referred to or co-managed with other healthcare providers.


3. Appointments & Cancellations

Appointments may be made online, by phone, or in person. To help us serve you and other patients efficiently, we ask that you:

  • Arrive on time for your appointment
  • Provide at least 24 hours’ notice for cancellations or reschedules
  • Understand that repeated missed appointments may result in fees or dismissal from care
  • Be aware that late cancellations and no-shows will incur a fee

4. Payment & Insurance

Payment is due at the time of service unless prior arrangements have been made. We accept credit/debit cards, HSA/FSA accounts, cash, checks, and money orders.

  • We do not bill Medicaid, Tricare, or in-network with any insurance policy
  • Your bill is due in full by 36 weeks gestation

5. Informed Consent

Before receiving care, you will be required to complete intake forms and provide informed consent. This process includes understanding:

  • The type of care you are receiving
  • The potential benefits and risks
  • Any available alternatives

You may decline or discontinue care at any time.


6. Privacy Practices

We are committed to maintaining the privacy and security of your personal health information. Our policies comply with applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA). Please refer to our Privacy Policy for more information.


7. Website & Digital Content

Our website is intended to provide general information on wellness topics. It is not medical advice and should not be used to diagnose or treat any condition.

You may not:

  • Reproduce or distribute any part of our website without written permission
  • Use the website to transmit harmful, unlawful, or inappropriate content

8. Limitation of Liability

To the fullest extent permitted by law, Sol Midwifery Services, including its employees and affiliated providers, is not liable for any indirect, incidental, or consequential damages arising from the use of our services or website.

Please also note:

  • Sol Midwifery Services is not responsible for the conduct or services provided by independently operated businesses or tenants leasing space within our building
  • We make no guarantees regarding specific outcomes. Results vary by individual

9. Modifications to Terms

We may revise these Terms from time to time. When changes are made, the “Last Reviewed and Updated” date will be modified accordingly. Your continued use of our services or website signifies your acceptance of the updated Terms.


10. Governing Law

These Terms are governed by the laws of the State of Indiana. Any legal disputes shall be handled by the appropriate courts located in Marion County, Indiana.
